• Consent Agreement

    The Congress of Neurological Surgeons ("CNS") values the privacy of its members ("you(r)" or "member"). CNS collects certain personal data and information ("Data") in the provisions of its "Services," including membership services, educational products, websites, and resources.

    This Consent Agreement specifies: (1) what Data CNS collects about you and your use of CNS's Services; and (2) how CNS uses or distributes the Data in connection with the Services.

    You may communicate this withdrawal by contacting CNS support at  or (847) 240-2500. Once you have withdrawn your consent, CNS will delete all records of your Data and personal information in its possession, to the extent it is technologically feasible to do so.

    This Consent Agreement shall be integrated in and form part of the Terms and Conditions. CNS may revise this Consent Agreement from time to time. The most current version of the Consent Agreement will govern your use of the Services. By continuing to access or use the CNS Services, you agree to be bound by the revised Consent Agreement.

    Personal

    CNS collects the following personal information: name; physical address; citizenship; email address; telephone number; fax number; member identification; login credentials; and birthday.

    CNS uses this information:

    • To manage the CNS products and services;
    • To facilitate CNS's administrative, financial, regulatory, and business purposes;
    • To provide customer support;
    • To correspond with you about CNS;
    • To facilitate educational programs, webinars, seminars, and meetings provided by CNS or third parties;
    • To provide marketing materials, including about CNS webinars, seminars and meetings and about third party educational programs; and
    • To maintain a Member Directory, participation in which is voluntary.

    CNS may sell your name and physical address to third parties as part of a member mailing list. CNS does not include your email address or other personal information in mailing lists shared with third parties.

    Professional

    CNS collects the following personal information: medical school training; residency training; board certifications; practice type; curriculum vitae; professional references; and personal photographs submitted with membership applications.

    CNS uses this information:

    • To review membership applications;
    • To manage the CNS products and services;
    • To facilitate CNS's administrative, financial, regulatory, and business purposes;
    • To provide customer support; and
    • To maintain a Member Directory, participation in which is voluntary.

    Financial Transactions

    CNS collects the following personal information: membership dues payments; product purchases; and ecommerce transaction history (product, payment amount, and PayPal transaction ID).

    CNS uses this information:

    • To manage the CNS products and services;
    • To facilitate CNS's administrative, financial, regulatory, and business purposes;
    • To provide customer support; and
    • To provide marketing materials, including about CNS webinars, seminars and meetings and about third party educational programs.

    Donations

    If you provide a charitable donation, CNS collects the following personal information: donor records; including amount donated; name; address; email; and tax deduction information.

    CNS uses this information:

    •  For CNS's internal business purposes;
    • To correspond with you about CNS; and
    • To provide you with tax deduction information related to your donation.

    Educational Programs

    CNS collects the following information: attendance; related financial transactions; duration (for claiming credit); test performance (where applicable); practice type; age; geographic location; and survey feedback. 

    CNS uses this information:

    • To facilitate educational programs, webinars, seminars, and meetings provided by CNS or third parties;
    • To analyze the effectiveness of the educational programs, and enhance the product offerings;
    • To provide marketing materials, including about CNS webinars, seminars and meetings and about third party educational programs; and
    • To share with Crowd Wisdom, the third-party vendor CNS uses to provide its educational programs.

    Meetings, Seminars, and Webinars

    If you participate as faculty or a presenter, CNS collects the following: name, physical address and potentially other personal information (see above); and, scientific research and papers.

    CNS uses this information:

    • To facilitate educational programs, webinars, seminars, and meetings provided by CNS or third parties;
    • To maintain the Abstract Center, where scientific research and papers are available to members; and
    • To share with share with ATIV Software, the third-party vendor CNS uses to provide application software for its meetings.

    Correspondence

    CNS collects the following information: email communications and other correspondence with CNS.

    CNS uses this information:

    • To manage the CNS products and services;
    • To facilitate CNS's administrative, financial, regulatory, and business purposes; and
    • To provide customer support.

    Marketing Materials

    CNS collects the following information: name; email address; opens, clicks, and click-throughs.

    CNS uses this information:

    • To manage the CNS products and services;
    • To facilitate CNS's administrative, financial, regulatory, and business purposes;
    • To provide customer support;
    • To correspond with you about CNS;
    • To facilitate educational programs, webinars, seminars, and meetings provided by CNS or third parties;
    • To provide marketing materials, including about CNS webinars, seminars and meetings and about third party educational programs; and
    • To share with MailChimp, the third-party vendor CNS uses to facilitate its mailings and marketing materials.

    Website Use

    If you use the CNS website and member portal, CNS collects the following information: IP address; cookies; web beacons; geographic location; member identification; clicks, page views, and time and duration of visit.

    CNS uses this information:

    • To manage the CNS products and services;
    • To facilitate CNS's administrative, financial, regulatory, and business purposes;
    • To provide customer support;
    • To analyze website engagement, and to develop and enhance the CNS website; and
    • To share with Google Analytics, the third-party vendor CNS uses to analyze website use.
